Before coming into our office for your first appointment, please print and fill out the New Patient Registration and Health History Forms located here. We also ask that you read the HIPAA Policy so that you are aware of the guidelines and practices surrounding the collection, storage, and use of patient information in our office. In addition to the completed New Patient forms, please bring your insurance card and ID to your initial visit as well.
For your first visit, we generally try to reserve 90 minutes specifically for you. We use this time to get to know you, review your medical history, and become familiar with your dental goals. Of course, it’s also time for you to get acquainted with us and ask us any questions you may have.
During a typical first visit, we determine if there are any necessary X-rays you’ll need, perform an oral exam, including oral cancer screening, and thorough periodontal evaluation. We determine what your needs are, and if possible, the hygienist will complete a dental prophylaxis (teeth cleaning). We will discuss our findings with you and outline any next steps for your personalized treatment plan.
Yes! As a family-friendly dental practice, our team works with children of all ages, from infants and toddlers all the way through pre-teens and teens. Our staff is friendly and welcoming, and we use fun, kid-friendly explanations to help children understand what’s going on with their dental health. Through frequent cleanings and exams, we will help your little one develop and maintain a healthy smile while also teaching him/her how to brush and floss effectively in between appointments.
